How to enrol

It’s easy. Visit our Health Professionals forms section and download the application form and print it out.

Please complete the application form and send it with your payment to NZ Medical Professional Indemnity Insurance (NZMPI). We will then provide you with:

• Your personal copy of the policy wording
• A tax receipt
• A medico-legal assistance card
